                                    Announcing the opening of Individualized Care Management LLC
Individualized Care Management LLC is an independent professional geriatric care management company based in Saline, Michigan and serving the Washtenaw county area of southeastern Michigan. This agency provides clients the option of working with an independent care manager that is able to interact with all of the different facilities and care providers in the area and is not tied to a specific company. 
“I first experienced this approach to providing care when I worked with Ingrid Deininger and Lois Jelneck at Individualized Home Nursing Care over 10 years ago. I look forward to being able carry on their tradition and offer the same high level of care to older adults in this community, wherever they call home” says Nancy Swierz founder and President of Individualized Care Management, LLC.
Individualized Care Management, LLC is directed by Nancy Swierz, MSN, RN, CMC, who is a licensed Registered Nurse as well as a Certified Care Manager. She holds both Bachelor's and Master's degrees in Nursing and has worked as a Registered Nurse since 1986. She has focused her nursing practice on providing care to older adults in their homes since 1989 and has been a member of the National Association of Professional Geriatric Care Managers since 2000. This extensive professional background allows her to effectively aid clients in navigating the complex care needs of an individual with a complicated medical history.
A Geriatric Care Manager is a specialist who helps families and individuals who are assisting in caring for older adults. This professional assists older individuals and persons with disabilities in attaining their maximum functional potential.  In addition, the Geriatric Care Manager is an experienced guide and resource for families of older adults and others with chronic health challenges. “In essence, Care Managers help families solve complicated elder care problems and provide both support and peace of mind along the way” summarizes Nancy Swierz.
