Individualized Care Management LLC is directed by Nancy Swierz MSN, RN, CMC who is a licensed Registered Nurse as well as a Certified Care Manager. She holds both Bachelor's and Master's degrees in Nursing and has worked as a Registered Nurse since 1986. She has focused her nursing practice to providing care to older adults in their homes since 1989 and has been a member of the National Association of Professional Geriatric Care Managers since 2000.

Additionally, for the last 22 years Nancy has specialized in the provision of certified home care services concentrating in providing leadership in Certified and Private Duty Home Health program start-ups and turn arounds, Program development, Process improvement, Policy analysis, Regulatory compliance, Team building, Operational analysis and Strategic planning.
